I: PREPARING THE APPLICATION
Only ONE Application per person
Use plain paper and type or clearly print in the English alphabet
The application MUST be signed by the APPLICANT
Include the following information in the Application:
(1) Your full name with last/surname underlined:

(2) Your date and place of birth:

(3) Your native country, if OTHER than your country of birth

(5) Full Mailing Address (Telephone Number if Available)
(6) Recent Photographs (less than 6 months old) of the Applicant,
the Spouse and ALL children (including legally adopted and stepchildren)who
are unmarried and under the age of 21
1.5 inches to 2 inches (37-50 mm) square in size
Applicants names and dates of birth must be printed on the back
The persons in the photos must be facing forward, covering 50% of
the area of the photo, in front of a light backgroun, face must be
in focus, no hats or dark glasses, only religious head coverings allowed
but not obscuring the face.
Attach the photographs to the entry with clear tape
(7) Signature of the Principal Applicant
This MUST be signed (with a full signature) by the applicant, otherwise
the application will be rejected
PART II: MAILING INSTRUCTIONS
Send the application by regular postal service mail: do not send by
certified or registered mail, Fedex, etc.
Send the application in an envelope which is between 6 inches and
10 inches (15 cm to 25 cm) in length and between 3 1/2 inches and
4 1/2 inches (9 cm to 11 cm) in width.
No postcards
Envelope return address: In the upper left hand corner must
list the country of which the applicant is a native, followed by the
applicant’s full name and return address, including the country of
address, even if the country listing is the same. See the example
below:
